MCN Diabetes Initiative:

The Migrant Clinicians Network Diabetes Initiative collaborates with health centers and social service agencies across the nation to better address the burden of diabetes in the migrant population.

The Main Goals of the MCN Diabetes Initiative are:

  • Increase the capacity of clinics and community organizations to address diabetes within mobile populations.
  • Improve continuity of care for mobile underserved individuals with diabetes.
  • Increase understanding and awareness of self-management amongst migrant workers.
  • Increase awareness and knowledge of diabetes prevention and care in the community of migrant workers, their health care providers and the public opinion in general.

MCN Diabetes Initiative Projects:

  • Alianza initiative consisting of six migrant health clinics working together in partnership to implement positive changes within their organizations.
  • The Migrant Diabetes Coalition, which represents various community sectors and is focused specifically on migrant concerns, guides the work of the Diabetes Program.
  • Diabetes Track II functions under the umbrella MCN Health Network, and is used to increase continuity of care for mobile underserved individuals with diabetes by offering bridge case management and self management support.
  • Diabetes education, awareness and prevention campaigns supported by the creation and distribution of culturally and linguistically appropriate educational materials.

For more information, see the MCN handout Why is Nutrition Important? which states the purpose of maintaining a balanced diabetic meal plan and what nutrients are in the food we eat. You can also use the MCN Nutrition Small Changes handout for suggestions about small nutritional changes you can make to your diet and why they are healthier. All of the MCN Diabetes Initiative materials are designed specifically for persons with diabetes, keeping in mind the issues of balanced nutrition, self management, healthy living activities, and the importance of regular physical activity to maintain healthy blood glucose levels.
