Continuous Prenatal Care for Mobile Patients
Introducing a project that will work to ensure continuity of care and provide bridge case management and medical records transfer for mobile pregnant patients when they move between health care providers.
Continuity of care is one of the greatest challenges of providing health-care services to migrant and mobile underserved patients.
When the mobile patient is a pregnant woman, clinicians are especially frustrated by interruptions in care. Not only does mobility represent a challenge and a frustration to those who provide pregnancy care, but the impact on pregnancy outcomes could also be significant. We base pregnancy management on the assumption that women who receive early and regular prenatal care are more likely to have healthier infants.
Beginning Feb. 1 2006 any prenatal patient who you suspect may move out of your area before giving birth is eligible to be enrolled in the Heatlh Network. We will provide care coordination throughout the course of her pregnancy, up until her postpartum visit, to help ensure that there are no gaps in her health care.
We are excited to work to ensure continuity of care for mobile pregnant women, and encourage clinicians to enroll all their patients who are pregnant and mobile!
