MCN addresses cancer in the migrant population through three successful initiatives that work in concert with one another.
MCN’s Health Network [1] provides clinicians with a unique opportunity to provide a patient centered medical home for their patients who move frequently (usually because of work) by providing a system that allows them to locate their patient and improve their continuity of care. At the same time Health Network provides patients and their families with personalized referral to care sources, information and on-going support as they move from one location to another throughout their course of treatment and beyond.
CPRIT (Cancer Prevention Research Institute of Texas [2]) funded MCN’s Moving Against Cancer initiative to conduct a state-wide program aimed at improving cancer screening rates, entry into treatment and survivorship among underserved Texas Latino communities. MCN is partnering with Ventanillas de Salud [3] and Migrant/Community Health Centers throughout Texas to promote cancer education and prevention.
LIVESTRONG [4] (previously the Lance Armstrong Foundation) has contracted with MCN to collaborate with Ventanilla de Salud across the United States to train their promotores de salud on the most common physical, emotional and day-to-day concerns of cancer survivors. As of September, 2011 promotores trained in cancer survivorship have reached out to 1252 individuals in the Latino/Hispanic Community with education about cancer survivorship.
