What is Diabetes Track II?

Clinics which treat mobile patients with diabetes can now count on a clinical oversight system that can help prevent diabetes-related complications by providing self-management support, education and referral.

Diabetes Track II LogoTrack II is a project of the MCN Diabetes Initiative, and is designed to track mobile individuals with diabetes to encourage continuity of care with the transfer of vital health information between clinics, assistance to finding and accessing services, and support as they implement their treatment plan.

Clinicians enroll mobile patients by obtaining a signed Health Network consent form for the release of private health information, and transferring the patients current medical records to the Migrant Clinicians Network for inclusion in a secure database. Access to patient data is limited to treatment and tracking for diabetes. Only MCN and providers involved in this treatment have access to this data. MCN does not share this data with anyone who is not involved in a patients' treatment or tracking.

Once a patient moves, clinicians in the receiving sites will be able to access that patient's existing medical record. By simply calling MCN's toll free line, the patient's information will be at the clinician's fingertips, which includes medical histories, previous treatments, and lab results. Results from foot, dental and eye exams will also be available. This information will allow clinicians to assist with contingency planning so they can inform their colleagues in other locations that a patient needing assistance could be headed their way.
