About the Series

MCN is pleased to announce our upcoming community of practice series, Diabetes ECHO 2027 for community health workers/promotores de salud, case managers, health educators, outreach workers, and patient navigators who work with health centers. This six-part, Spanish-language series will address topics relating to diabetes, including general knowledge, nutrition, mental health, complications, and more.

The ECHO Diabetes series consists of six 90-minute sessions, and for the first time, each session will integrate the effects of climate change and weather-related phenomena as it relates to successful Diabetes Self-Management Education and Support (DSMES). Session topics include: a general introduction to pre-diabetes & overview of Type 2 Diabetes (T2D); mental health & chronic disease; complications, treatments & medications; nutrition & exercise; and gestational diabetes. Individual sessions will focus on those key elements that make successful management challenging within the specific session topic, and how social determinants of health — the environmental and social conditions that we are exposed to where we live and work — exacerbate successful control particularly in underserved communities. Participants will be provided with strategies and best practices to address the numerous challenges patients face as they learn to adapt to living with and successfully managing T2D.
