Description

Evidenced-based research demonstrates that patient engagement leads to improved health outcomes and quality of life. To effectively engage patients in their health care, health centers can utilize a team-based approach to develop a shared care plan and support patients as they navigate the health care system. In this six-part interactive series, Migrant Clinicians Network (MCN) and National Nurse-Led Care Consortium (NNCC) will work with a select group of ten health center care teams to improve knowledge and skills around evidence based strategies for patient engagement and activation.

Participants in the virtual learning collaborative will attend six (6) learning sessions consisting of live web-based platform training, offline team-based activities, and facilitated office hours. These learning sessions will be hosted from January through June of 2019 and will take place via the Zoom video conferencing platform. CEs will be provided. All modules will be presented/facilitated by staff from MCN and NNCC unless otherwise noted. All modules will take place at 1 pm ET.

MCN will provide CME and CNE credit at no cost to participants by the American Academy of Family Physicians and the American Nurses Credentialing Center’s Commission on Accreditation. Determination of credit is pending.
