Join Migrant Clinicians Network (MCN) presenting on Health Network, a virtual care coordination program, connecting mobile patients with linkages at a new health center in their next location along with medical records transfer and other guidance to ensure health care access and treatment completion. In this webinar, clinicians are introduced to Health Network and how to use MCN's secure Health Network Portal, which allows clinicians at health centers to easily enroll patients in Health Network and follow their progress.
At the conclusion of this presentation, participants will be able to:
- Learn about Health Network and how our Health Network Associates can help your health center coordinate care for your mobile patients.
- Begin enrolling your health center’s mobile patients in MCN’s HN Portal by taking the first step and learning how to create a Portal account.
- Learn and understand the Portal workflow including steps to upload documents that will help facilitate care coordination for any of your health center’s mobile patients.
- Access and view any previously uploaded documents with the most recent patient information.

Saul Delgado is a highly motivated professional with a diverse background in migrant justice at the crossroads of health care and technology. In 2011, inspired by his mother, a dedicated nurse in Cuba, Delgado enrolled at Southern Careers Institute, graduating with specialization as a Medical Assistant. He then assumed the role of a Medical Assistant Floater at the Austin Diagnostic Clinic. There, he demonstrated skills in managing varied responsibilities across the Pulmonary, Cardiology, and Nephrology departments.
In 2013, Delgado joined Migrant Clinicians Network, where he assisted migrants with ongoing health needs to find care in their next location, as a Health Network Associate. Over the years, Delgado has exhibited a keen aptitude for technology, assuming significant responsibilities in the development and customization of the Health Network database. After working as MCN's Health Network Data Specialist, he has advanced to become MCN's Data Operations Analyst I. His expertise has played a vital role in optimizing the database to cater specifically to the needs of enrolling clinicians and their Health Network Associate partners, thereby enhancing the efficiency and impact of their work.
Delgado's professional trajectory showcases a commitment to excellence, a blend of medical proficiency, and technological acumen. His strategic contributions in health justice and health technology underscore his dedication to advancing both fields and making a positive impact on the community he serves.

Alma Colmenero (she/her/ella) is the Prenatal Coordinator for Health Network, providing bilingual and culturally competent case management to hundreds of pregnant migrants to ensure they receive the prenatal, birth, and postpartum care they need. Colmenero began her Migrant Clinicians career in 2018, as a Health Network Associate, managing a wide range of cases until she advanced to Prenatal Coordinator in 2021 in recognition of her management successes and dedication. Colmenero holds a bachelor’s degree in Business Administration – Marketing from the University of Texas, Austin. She was born in San Luis de la Paz, Guanajuato but has lived most of her life in Dallas, Texas before moving to Austin to pursue her undergraduate degree; she and her siblings are the first generation in their family to receive a college education. When she’s not working, Colmenero likes to watch movies and go to the pool. She has two young daughters and two dogs, a chihuahua named Tequila & a Cane Corso named Chevy, who love to be outdoors when it’s not too hot.

Brenda Ramirez, MCN’s Field Health Network Associate in McAllen, Texas, is an experienced case manager. After graduating with a Bachelor’s degree in Psychology from the University of Texas, Rio Grande Valley, she joined MCN's Health Network team. Working directly with certified nurse midwives at migrant shelters in Texas, she specializes in prenatal and postpartum case management, helping pregnant migrants enroll in MCN’s Health Network program.
Bilingual in Spanish and English, Ramirez works directly with Health Network patients at immigration shelters, providing culturally contextual support and enrolling the patients in Health Network. After the patients leave the shelter, she provides bilingual and culturally contextual case management as they travel to their destination states, assisting in making appointments and navigating the complex health care system that can be a barrier to care for migrant patients. By getting to know many of our patients more directly, she helps the rest of the Health Network team stay aware of the most pressing concerns and issues migrants face when entering the country, helping the team keep their finger on the pulse of border communities and migrants.
Ramirez hopes to attend graduate school in the future for counseling. In her free time, she enjoys spending time with her husband and family, their four dogs, being outdoors, and listening to a true crime podcast.
