What is Health Network?

Health Network provides patient navigation and care coordination, including medical records transfer, peer support, and follow‑up support, for patients who leave their service area. This important and effective service helps ensure continuity of care and supports a healthy agricultural workforce as the season progresses. Health Network also assists clinicians, community‑based organizations, and public health services in better serving patients after a disaster or other emergency, as people temporarily or semi‑permanently leave their community and arrive in a new one.

Health Network has proven to be an easily modifiable care coordination system with applications in disease surveillance and treatment management across a wide range of injuries, illnesses, and care needs. Independent research on tuberculosis treatment has found that Health Network is highly cost effective

Health Network does not provide direct clinical care; instead, it coordinates care and supports patients and providers through transitions.

 

Who can enroll?

Enrollment must be initiated by a clinician, Community Health Worker, outreach or shelter worker, attorney, social worker, or partner organization and is free for the enrolling organization. At this time, patients cannot enroll themselves. Health Network provides patient navigation and care coordination services at no cost to the patient.

 

What is required?

The 3 things required for enrollment are:

  1. CONSENT FOR RELEASE OF MEDICAL INFORMATION (signed and dated)
  2. PARTICIPANT INFORMATION (including the patient’s working phone number)
  3. MEDICAL RECORDS and/or summary of next steps

These three documents constitutes the Patient Enrollment Packet.

How to refer a patient to Health Network?

The enrollment process depends on the type of organization referring the patient. All enrollment methods provide the same patient navigation and care coordination services.

Only available for Health Centers

Use Our Health Network Portal:

  1. You can use our Health Network Portal to enroll the patient electronically or email hn@migrantclinician.org to get an account started.
  2. Once we receive your Portal enrollment, we will verify the information and initiate continuity of care services. It is critical that you fill out Portal enrollment as completely as possible so that MCN staff can provide the best possible service to you and your patients. We recommend enrolling patients in anticipation of their departure; having a patient enrolled ahead of time will enable us to direct that patient to their next clinic before the patient leaves your area. Only patients enrolled will be followed. Health Network has been designed to facilitate the continuity of care for patients, not only in the US, but around the world.

Please note: At this time, the Health Network Portal is only available in English. 

Available for Health Centers, community-based organizations and other partners

Health centers, community‑based organizations and partner organizations (including Community Health Workers, outreach or shelter workers, attorneys, social workers, and public health partners) may enroll patients using the following options:

  1. Email – Download and Complete the Enrollment Form below. Have the patient sign it and submit it with medical records and/or summary of next steps (may include clinical notes, care coordination notes, or referral context). Send encrypted email to: hn@migrantclinician.org.
  2. Fax – Download and Complete the Enrollment Form below. Have the patient sign it and submit it with medical records and/or summary of next steps (may include clinical notes, care coordination notes, or referral context). Fax it to MCN (Medical and confidential fax: 888‑491‑2194).
  3. Phone – Call 512-327-2017 (with the patient present), we interview the patient, explain the program to the patient, we fill in the Patient Enrollment Packet for you and send them to you. Have the patient simply sign and then fax those back to us along with all of the patient's medical records.
